PARENT AGREEMENT: I hereby certify that is in normal health and capable of participating in soccer. I'm aware that the goals and the objectives of the U.S. BRAZILIAN SOCCER STYLE and sports programs are based on Fun, Fair Play and Skills Development. I'm aware that the U.S. BRAZILIAN SOCCER STYLE only carries a secondary health insurance and that my insurance bears primary responsibility. I also understand that U.S. BRAZILIAN SOCCER STYLE retains the right to use, for publicity and advertising purpose, photographs of players taken at the School or any event by the U.S. BRAZILIAN SOCCER STYLE.
